Medication Reviews

 

If you take repeat medication, you will need a review once a year. This helps ensure your medicines are still suitable and working well for you.

How the review works

  • You may be asked to complete an online questionnaire and/or book appointment with a GP or another qualified health professional (such as a Clinical Pharmacist)
  • Reviews can take place by phone or in person
  • The due date for your review appears on your repeat prescription slip.

Before your review

  • We will send you a medication review questionnaire. This helps us decide whether you need an appointment and what type.
  • Sometimes we may ask you to have tests first (e.g. blood pressure checks or blood tests) to ensure your medication is working properly.

During the review

  • You will be asked how you are getting on with your medicines.
  • Please share any problems, side effects, or concerns.
  • You can ask any questions you may have.
  • Any changes to your medication will only be made with your agreement.
  • A record of the review is added to your medical notes.

Important

  • If you do not complete the questionnaire or book your review when requested, your future prescriptions may be temporarily restricted until the review is completed. This is to ensure safe prescribing.
  • If this continues to happen, your prescription quantities may be reduced to 14-day supplies, or you may be asked to register with another GP practice.
  • Safe prescribing is essential for your wellbeing and for maintaining a good GP–patient relationship

How to prepare

  • Note the date, time, and name of the clinician you will be seeing or speaking to.
  • Make a list of all medicines you take, including:
    • Prescribed medicines
    • Medicines you no longer take
    • Over-the-counter medicines, vitamins, and herbal remedies

If you cannot make a list, bring all your medicines to the appointment or have them ready if your review is by phone.

  • Think about any questions you want to ask. You may want to know:
    • Why the medicine is important
    • When and how to take it
    • How long you need to take it for
    • How to tell if it is working
    • What to do if you have problems or side effects
    • Anything you should avoid while taking it
    • What happens if you miss a dose or stop taking it

After the review

  • Your Named GP will be informed of any medication changes.
  • A summary of the review will be recorded in your medical notes.
  • Any agreed tests or referrals to other healthcare professionals will be arranged.
